 Birth certificate This street has now disappeared and is covered by a shopping complex[3].  He was baptised at the Hardwerwijk Roman Catholic Church on 18 April.  Baptism entry - note spelling of his name compared with birth certificate Johannes Hendrikus lived for only 6 days. He died on 23 April at his parents' house at 3.00am.  Death certificate |
